Case Management Report v2

Access and use the Case Management Report v2.

The Case Management Reports provide a detailed breakdown of attendance, client engagement, and the usage of each section.

These reports can be filtered by date, outcome, hub, referral handler, and contact type, allowing for deeper analysis and more targeted insights.

Access the report

The Case Management Report v2 is an automatically generated report on the platform that provides all the information related to the case data.

To access this report:

  1. Click Reports in the left menu bar.

  2. Click Referrals/Cases.

  3. Click Case Management Reports v2 (New).

Report details

You can see the five tabs that the report is broken up into. Each section of the report displays different information that you can report on.

Appointments

  • Which referral handler created the appointment.

  • What the contact type was for the appointment, for example, telephone or drop-in.

  • What was the outcome of the appointment, for example, attended or rescheduled.

  • Which hub the appointment was made in.

Visits

  • Which referral handler created the appointment.

  • What type of visit was it, for example, a letter or telephone.

  • What was the outcome of the visit, for example, contact made or message left.

  • The time spent with the client.

  • Which hub the visit was created in.

Goals

  • Which referral handler created the goal.

  • The status of the goal.

  • If their goal has a due date.

  • Which hub the goal was created in.

Prescriptions

  • Which referral handler made the prescription.

  • Is the prescription active.

  • What the outcome of the prescription is, for example, attended or not yet attended.

  • The rating of the intervention.

  • The primary and sub-domains used for each intervention.

  • How many interventions were prescribed by each hub.

Sign Postings

  • Which referral handler made the sign posting.

  • Is the sign posting active.

  • What is the outcome of the signposting is.

  • The rating of the sign posting.

  • The primary and sub-domains are used for each sign posting.

  • How many sign postings by each hub.

All of these tabs also contain a time bar. This can be used to filter your report to the time frame of your choice.

Within each tab, the data is broken down into two further tabs:

Monthly Overview

Shows a graph, giving you a high-level overview of the section.

Details

Shows further details of the section, allowing you to drill down further.

Each section can also be filtered down using the Filters menu on the left-hand side. The filter details change, depending on which section you've selected.
