Send a referral for a client to a hub in another licence. This allows services to work together where referral pathways have been enabled.

Before you start

⚠️ Important:

  • Referral pathways must be enabled between licences.

  • Both projects must agree to share referrals.

  • The Access Elemental team must configure this for you.

📌 Note: Only hubs with open referral pathways will appear when creating an onward referral.

Create an onward referral

  1. Go to Client Tools.

  2. Select Find Client or Find Case.

  3. Search for and open the client.

  4. Click Show Cases, then select View Case.

  5. In the Manage Client Case screen, click Case Actions.

  6. Select Onward Referral.

Select the referral destination

  1. Choose the hub you want to refer the client to.

    • Only hubs in other licences with enabled pathways will be shown.

  2. Click Create Onward Referral.

Confirm referral source

  1. When asked Are you referring on behalf of someone else?, select No.

  2. Select your connected hub.

  3. Click Proceed to Referral Details.

Complete the referral form

  1. Review the pre-populated client details.

  2. Check all information is accurate and appropriate to share.

  3. Update the reason for referral as required.

    • This field auto-fills with the most recent referral reason, update it before submitting.

📌 Note: Always ensure only relevant and necessary client information is shared.

Submit the referral

  1. Tick Client Agrees to confirm consent has been given.

  2. Click Generate Referral.

The referral will then be created and sent to the selected hub.
