Track the outcome of an intervention

Update and monitor the outcome of interventions directly from a client’s case data.

You can track the outcome of an intervention you have signposted or referred a client to.


Step by step guide

  1. Go to Client Tools.

  2. Click Find Client.

  3. Click Show All.

  4. Enter the client’s name in the search box.

  5. Under Actions, click Case Data.

  6. To update the intervention outcome, click either:

    • Manage Interventions, or

    • The Social Prescription tab.

  7. On the Outcome list, select the appropriate option to reflect the result of the intervention.
